Transition

Helping students and families prepare for the transition to life beyond high school is a priority at The Foundation High School. Following are descriptions of the range of options that go into each student’s transition planning. Every student has an Individual Transition Plan as part of the Individual Education Plan that is implemented through a customized combination of the following classes and supports.
